Dark Ritualist's ability only triggers if Dark Ritualist dies and goes to the graveyard. Removing it from the game with a card like Smite or Apocalypse Stone will not trigger the ability.

The Demon Spawn doesn't return to the side deck after being killed, so you effectively only get one use of the death trigger per game.

You cannot destroy Dark Ritualist in response to the trigger to negate the ability. You can however remove it from the game or cause it to lose its abilities to negate the effect.
