If your turn starts and you have Cursed Collar equipped, you will not have a chance to use its active ability before the damage trigger.
The active ability costs 2 mana (M) and 2 health (H). Spending health for an ability is not damage and cannot be prevented.
Spending health (H) for the active ability can kill your Hero.
If you have multiple Heroes, the Hero that Cursed Collar is equipped to must spend the health (H) for the active ability.
If the active ability is negated in response to its activation, then you still have to pay the mana (M) and health (H) cost.
The active ability can be negated in response by either destroying Cursed Collar or removing it from the game.
